Employee Account Access Form

I am curious if any other institutions have their staff sign a form if they are granted access to view other employee accounts.  We have recently encountered staff members with access to view employee account information looking at multiple employees on the bonus payroll day.  Currently, we have nothing that the employee signs stating that employee accounts should only be accessed for business needs.  Would anyone have something they would be willing to share or items to include in this form? (Please feel free to copy and paste, even if the formating is off so everyone can enjoy the information.)
